The cause of the crash is still under investigation. The committee says it has previously raised concerns about train safety, staffing, filling essential posts and how incidents reported by workers are handled.

It wants specific answers, enough staff and technical resources, effective prevention and properly functioning groups to involve staff and monitor safety concerns. The committee says it will keep pressing for safety to take priority for both rail workers and passengers.
